Imagine a world where heart patients no longer face long waits and distant hospital visits for specialized care. That's the reality for residents of East Galway, thanks to a groundbreaking initiative called the Integrated Care Hub. But here's where it gets even more impressive: this community-based program, led by Consultant Cardiologist Dr. Nuria Farre, is not just about convenience; it's about transforming cardiac care into a personalized, accessible, and empowering experience.
Since its launch in April 2023, the East Galway Roscommon Integrated Care Hub has become a lifeline for over 1,000 patients. Operating across towns like Ballinasloe, Tuam, Loughrea, and Mountbellew, as well as Castlerea and Monksland, the Hub caters to individuals aged 16 and above with conditions such as atrial fibrillation, hypertension, high cardiovascular risk, complex dyslipidaemia, heart failure, or a family history of premature ischemic heart disease. And this is the part most people miss: it's not just about treating the condition; it's about educating patients, managing medications, and providing comprehensive cardiac rehabilitation—all within their local communities.
The Hub’s success is evident in stories like that of Bernadette Whelan from Monivea. After a cardiac event and treatment at University Hospital Galway, Bernadette was referred to Dr. Farre and her team. “From the very first meeting, I was treated with warmth, compassion, and professionalism,” she shared. “They supported me every step of the way, even while I was on holiday, and made me feel truly cared for, not just like another patient.” Her experience highlights the Hub’s patient-centered approach, which goes beyond medical treatment to foster a sense of trust and empowerment.
The multidisciplinary team at the Hub includes a Consultant Cardiologist, Advanced Nurse Practitioner in Heart Failure, three Cardiology Clinical Nurse Specialists, a Cardiac Rehab Coordinator, a Senior Physiotherapist, a Cardiology Staff Nurse, and administrative support. Together, they deliver timely specialist care, patient education, and cardiac rehabilitation, ensuring that patients receive holistic support tailored to their needs.
Here’s where it gets controversial: While the Hub has significantly reduced delays in access to specialist care and eased hospital pressures, some might argue that such community-based models could dilute the quality of care. However, Dr. Farre counters this by emphasizing the Hub’s role in empowering both patients and GPs. “Our Clinical Nurse Specialist team empowers patients to manage their conditions confidently while ensuring they receive timely expert guidance,” she explains. This model, she adds, is a core component of Sláintecare, Ireland’s strategy for reforming the health and social care system.
Initially focusing on patients from Portiuncula University Hospital waiting lists, the Hub has since expanded to include GP referrals. This shift means patients who would have traditionally been sent to hospitals are now being treated directly in their communities, improving access and outcomes.
